Drivers License Renewal

I need to renew my drivers license soon. I understand that there will be a questionaire about my health.
I'd love to see a sample, anyone know where I could find one?

You didn't mention where you live which is important since there are different requirements for the city and the provincia. If I remember correctly, you live in La Plata.

It seems you will be examined on site when you go for the renewal appointment.

My 84 year old mother in law didn't have any problem renewing her license and she's like the walking dead....dodgy heart,eyesight,coordination. Last time she went out in her car she caused mayhem in the supermarket car park!!
