I don't know how I had never heard about this movie, but I recently watched it and loved it. Whiskey Romeo Zulu (2004) is about the 1999 crash of LAPA flight 3142 at Aeroparque. I fly planes and while watching it I was noticing how accurate many of the aviation scenes were, which is rare for movies. The movie goes into the root cause of the crash that killed 65 people when the Boeing 737 tried to take off without flaps. The immediate cause was that the flaps weren't configured for takeoff, but the pilots also ignored the alarm telling them that the flaps weren't set! Why did they ignore the alarm? Because they had gotten used to false alarms because of the crappy maintenance - "normalization of deviance" as it's called in the aviation world.

After I watched the movie, I realized that the actor who plays the main character is actually playing himself - he was the pilot who tried to blow the whistle on the airlines before the crash. Anyway, if you're into this kind of movie, I highly recommend it. It's on youtube, but without subtitles. If you want subtitles, the only place I found is Apple TV. Here's the youtube link:
