I'm going to bump this to see if over a year later we have learned about more options. Lunar's Diginet suggestion here was excellent, but the Diginet outbound number changes your number to private and possible spam, which is a bit inconvenient for your friends and contacts receiving your call.

Does anyone have any additional services to recommend?
