Has anyone received the SS proof of life form yet?

The forms include the notice that if not returned in 60 days that the payments will be suspended. My SS payments have been suspended three times because the form had not been recieved.

I always recieved the January payment on the third (as promised), but when I checked My Social Security account online a day or two later I could see if future payments had been suspended.

Each time this happened I sent a PDF of the completed form to my sister-in-law in the USA . She printed and mailed the form to the appropriate address in Baltimore. The February payments were then credited to my US bank account on the third day of the month.
 
It depends on where your last name falls in the alphabet. I received the form in 2025. I was able to rely on a tango friend to mail it for me in the US, since Argentina doesn't accept US mail. She departed Buenos Aires on September 9, 2025, and mailed it Texas.
This may be the best way to ensure that SSA receives your reply.
 
How did you mail your form thru the Embassy? please give details. Tks
At that time US wasn't accepting any mail from Argentina. So I called USEmbassy (aware of this problem) and was instructed to send the form to them in a certain manner, wich I don't recall exactly, they will take care of it. So they did and it worked. I think I've got their phone # in some older post in this forum.
 
two years ago, had US embassy deliver my form. i would have preferred to use them again this year if they are still doing this service. several times i have mailed things to US from here in cordoba that never made it. but when i went on embassy web page, there was only a phone number as a way to contact them, and the phone number has consistantly been busy or unoperative....anyone used the embassy this year?
 
I've recently contacted the Federal Benefits Unit (FBU) at the embassy by email with a Social Security question, and they responded quickly (they usually do). This is the address I used:

They will ask for the following information, and including it in your initial email will expedite things:
Full name
Date of birth
Last four digits of the worker's social security number
Country of residence and complete address
One or more telephone numbers where you may be reached
Description of your request

Also, I mailed my signed 'Proof of Life' document to the embassy about 10 months ago without any issues. I noted 'Attention: FBU Proof of Live' below the embassy's address on the envelope so it could more easily be funneled to the correct office.
