Has anyone received the SS proof of life form yet?

I received today the Proof of Life Form SSA-7162 dated July 2, 2026.
 
I've recently contacted the Federal Benefits Unit (FBU) at the embassy by email with a Social Security question, and they responded quickly (they usually do). This is the address I used:

They will ask for the following information, and including it in your initial email will expedite things:
Full name
Date of birth
Last four digits of the worker's social security number
Country of residence and complete address
One or more telephone numbers where you may be reached
Description of your request

Also, I mailed my signed 'Proof of Life' document to the embassy about 10 months ago without any issues. I noted 'Attention: FBU Proof of Live' below the embassy's address on the envelope so it could more easily be funneled to the correct office.

I received it in July and was fortunate to be able to send it back with someone to the States to mail. Does this come every year? Every two years?
According to the SSA: Beneficiaries with social security numbers ending 00-49 must submit their enforcement questionnaire in even numbered years. Beneficiaries with social security numbers ending in 50-99 must submit their enforcement questionnaire in odd numbered years. Beneficiaries with a representative payee and 90 years or older must complete the form annually.
