Do you have to get good grades to get in or do you just have to apply and be able to keep with the curriculum? I plan to work in South America when I'm older and UBA seems like a really great university.

To become thorough in Castellano, enough to study medicine, give yourself a few years of intense Spanish exclusive studies. Ditto on the search function. There are many aspiring UBA students that have asked the same question and many have answered.
