I asked the U.S. Embassy FBU for help regarding this issue and, as usual, they make as little effort to be helpful as possible.

Here is the fax number which I used to send mine to SSA: 1-833-719-0632. I checked once and was told that my account is fine but I will call to check again to be sure.

Jantango, do you use your address here as the return address when mailing the form within the US? I'm such a paranoid about these things!
