I go to the Instituto Otorrinolaringológico Arauz. I've had no problems speaking Spanglish with several of the doctors. But you can request an appt with someone with fluid English.
I go to the Instituto Superior de Otorrinolaringología on Pasteur, and IIRC there are 1 or 2 docs who speak English that asked to practice their English with me (I didn't have the heart to tell them to use Duolingo after work )

I was there last year when I got hit with a nasty ear infection. The address is Pasteur 740. The service was very good.
One thing to keep in mind is that they only accept payments with debit cards. No CC. Or at least that’s what it was as of last November.
