Has anyone received the SS proof of life form yet?

I've recently contacted the Federal Benefits Unit (FBU) at the embassy by email with a Social Security question, and they responded quickly (they usually do). This is the address I used:

They will ask for the following information, and including it in your initial email will expedite things:
Full name
Date of birth
Last four digits of the worker's social security number
Country of residence and complete address
One or more telephone numbers where you may be reached
Description of your request

Also, I mailed my signed 'Proof of Life' document to the embassy about 10 months ago without any issues. I noted 'Attention: FBU Proof of Live' below the embassy's address on the envelope so it could more easily be funneled to the correct office.

You can log in to the SSA net site, downloaded, print it, fill it and mail it

i sent an email about this to embassy, they responded that the proof of life forms are being mail in august this year instead of may...


Received following from FBU Buenos Aires on June 25th.

"We would like to inform you that Form 7162 was sent by postal mail a few weeks ago, so you should receive it within the next 30 days.
If you do not receive it, please note that a second mailing will be sent in October to those who have not responded to the initial request.
If you have not received either mailing by mid-December, we kindly ask that you contact us through this channel. This will allow us to provide you with the necessary instructions to ensure your benefit is not suspended in February of the following year"
