Help - Need Emergency Passport Advice

My passport, along with my DNI, credit cards, and bank card was stolen over the weekend. I am going to the embassy tomorrow to request an emergency passport, and I need to know what I should bring. I can't really take more than one day off work for this, and so I want to make sure I arrive at the embassy prepared.

Has anyone been through this? Do you know what they want you to bring? Do I need a passport photo?

  • Photo identification, if available (i.e. valid driver’s license or military ID),
  • Proof of citizenship, if available (i.e. copy of passport, U.S. birth certificate or Naturalization Certificate),
  • One (1) 5 cm x 5 cm passport photograph,
  • Police report of the loss or theft,
  • Completed DS-11 Application for a U.S. Passport,
  • Completed DS-64 Statement Regarding Lost or Stolen Passport,
  • Passport fee.
