Are you up to dealing with intake procedures in Spanish in the clinics? I am fairly certain that the vast majority of the cardiologists in the major clinics like Favoloro, Fleni, Hospital Italiano, ICBA, would speak very good English. In ICBA, which I can totally recommend, various consultants spoke English with me with no prompting.

One problem with an independent cardiologist is likely to be a lack of equipment.

ICBA organize international conferences, and, until recently, were a destination for medical tourism. You would not go wrong with them.

Your problem is likely to be less with the cardiologists and consultants, and more with the secretaries and people who need to compile your data and arrange your appointments. No matter which route you go, you will probably have to deal with this.

Goofy generalizations.. Lots of doctors at the esteemed hospital Aleman don't speak English.

Silly old git. My comments about ICBA are from personal experience, in no way are they generalizations. But thanks for recommending where not to go.

There have been times and circumstances when even I have claimed not to be able to speak English but barring those types of situations, if I start a conversation in the other person's language, when I start to falter, the other person picks up. It's a natural human trait, to respond to a display of weakness rather than superiority. In those places where I genuinely don't know any of the local language I usually learn, parrot fashion, the phrases for "I apologise but I don't speak (language) may I speak to you in (list of languages I can speak) instead? That usually breaks down the barriers
 
IMO, having the best cardiologist (regardless of language) should be your first priority.

If the best cardiologist only speaks Spanish, then there are many interpreters in this country that can accompany you on your visits to help interpret for you.

I would not settle for less just to hope that his mastery of English is actually sufficient.
