Did he respond to the low level official who suggested Milei was under the influence of drugs? No, he went after the President of Spain and his wife. That's not defense, that's a tantrum at best and election interference at worst, in the worst way, accusing Sánchez and his wife of corruption.
Before the subject of Sanchez's support for Massa comes up, all he stated was that Massa represented moderation and unity, and the opponent represented harshness. That is hardly an attack, it was what Milei himself campaigned on, harsh measures.
Good summary. The Spanish minister in question also retracted his comment a few days later, thou he didn't apologize. That should have been the end of the affair.
About the "substance use", I believe Spain has an active intelligence service, and they will now be very motivated (or directed) to dig up dirt on Milei. A lot of it should stick.

The level of insults gets more and more infantile, as is to be expected, given the protagonist: "The president of Argentina, far from apologizing to the Spanish Executive, as the Government of Spain demanded, raised his tone this Monday by accusing the socialist president of being “cowardly” and “totalitarian” for believing he is “owner of the State.” Also, he noted that he “hides between women's skirts.”
Note that the same article states: "for now the Civil Guard has concluded in a report that there is no influence peddling on the part of the wife of the President of the Executive".
You can see why the Spanish are pissed off; even with Milei's diplomatic snubs, "during Milei's visit last weekend, Spain provided all the necessary means. It allowed him to land and take off from the Torrejón Base, offering safety guarantees. And the response was a frontal attack on [Spain's] institutions".
It does seem that Spain is considering declaring Milei PNG, something that would be entirely merited I think.
The usual right-wing gaslighting isn't going to wash. There's nothing reasonable about Milei's behaviour. My description of him as our "toddler president" seems to have grated on some, but it's not inaccurate.
