has anyone here actually ever had a citizenship case just completely fail? like impossible to get through? or know of one? if so, why?

Simple, the problem is always the judge. You can always quit and start it over asking for ab automatic change of judge (only a lawyer con do it).

When the process is initiated by an abogado, they can request ONE recusal without a "reason". The 1st judge can't say anything. It happens all the time and judges don't really care. You need to have an attorney to do that thou. If you want to change the judge AGAIN, you need a reason. You must demonstrate that the judge is "biased". This is dangerous territory and can make many people annoyed in all the wrong places.

Almost.
It is one recusal without expressing the reason, there is always a reason, by file. It means, if you loose the case, you can apply again in a new file with a new case number and recusal the former judge over and over until your citizenship is recognized.

so basically if it fails you can just keep filing the same case until you get a favorable judge?

You can request a change of the judge only ONE time without a reason, And only a lawyer can do that.

oof. new case means your witnesses come in again, you publish the decrees again, bring in all the paper work etc.

i'm like sweating bullets here hoping my case gets processed and positive outcome in march.
