Getting US Dollars from an ATM machine

I've seen numerous ATM machines offer the option of withdrawing US dollars, but I have never seen one actually dispense them.

Does anyone know of an ATM that will reliably give out US dollars?

If none of them do, then why am I even offered the "Pesos or Dollars" choice?

You have to have a dollar account with the bank in question to get dollars out of the ATM.
