OSDE is premier health insurance. They don't really care for your business ( I know it sounds harsh).But I have used them and they are best. Just take subte linea D ( Green). Get off at "Bulnes" station and their is a OSDE office right in front. Go with your passport or Dni and talk to them directly. They close by 6 pm.
