The interesting thing about the questionnaire is the wording: "...part of a religious ceremony." Being as the vast majority of males born in hospitals in the US between 1948(?) and the seventies were circumcised as a matter of course, many could answer that they were not and then be called out on the issue later, after an exam.

The gubment just gets better and better.
